"WASHINGTON - As part of Administrator Lisa P. Jackson's commitment to strengthen and reform chemical management, the U.S. Environmental Protection Agency (EPA) today announced a series of actions on four chemicals raising serious health or environmental concerns, including phthalates. For the first time, EPA intends to establish a "Chemicals of Concern" list and is beginning a process that may lead to regulations requiring significant risk reduction measures to protect human health and the environment. The agency's actions represent its determination to use its authority under the existing Toxic Substances Control Act (TSCA) to the fullest extent possible, recognizing EPA's strong belief that the 1976 law is both outdated and in need of reform."

Cancer and chronic fatigue syndrome (CFS) are both characterised by fatigue and severe disability. Besides fatigue, certain aspects of immune dysfunctions appear to be present in both illnesses. In this regard, a literature review of overlapping immune dysfunctions in CFS and cancer is provided. Special emphasis is given to the relationship between immune dysfunctions and fatigue. Abnormalities in ribonuclease (RNase) L and hyperactivation of nuclear factor kappa beta (NF-kappaB) are present in CFS and in prostate cancer. Malfunctioning of natural killer (NK) cells has long been recognised as an important factor in the development and reoccurrence of cancer, and has been documented repeatedly in CFS patients. The dysregulation of the RNase L pathway, hyperactive NF-kappaB leading to disturbed apoptotic mechanisms and oxidative stress or excessive nitric oxide, and low NK activity may play a role in the two diseases and in the physiopathology of the common symptom fatigue. However, in cancer the relation between the immune dysfunctions and fatigue has been poorly studied. Immunological abnormalities to such as a dysregulated RNase L pathway, hyperactive NF-kappaB, increased oxidative stress and reduced NK cytotoxicity, among others, are present in both diseases. These anomalies may be part of the physiopathology of some of the common complaints, such as fatigue. Further studies to confirm the hypotheses given here are warranted.

Recent research has discovered that a number of genetic risk factors for autism are de novo mutations. Advanced parental age at the time of conception is associated with increased risk for both autism and de novo mutations. We investigated the hypothesis that other environmental factors associated with increased risk for autism might also be mutagenic and contribute to autism by causing de novo mutations. A survey of the research literature identified 9 environmental factors for which increased pre-conceptual exposure appears to be associated with increased risk for autism. Five of these factors--mercury, cadmium, nickel, trichloroethylene, and vinyl chloride--are established mutagens. Another four--including residence in regions that are urbanized, located at higher latitudes, or experience high levels of precipitation--are associated with decreased sun exposure and increased risk for vitamin D deficiency. Vitamin D plays important roles in repairing DNA damage and protecting against oxidative stress--a key cause of DNA damage. Factors associated with vitamin D deficiency will thus contribute to higher mutation rates and impaired repair of DNA. We note how de novo mutations may also help explain why the concordance rate for autism is so markedly higher in monozygotic than dizygotic twins. De novo mutations may also explain in part why the prevalence of autism is so remarkably high, given the evidence for a strong role of genetic factors and the low fertility of individuals with autism--and resultant selection pressure against autism susceptibility genes. These several lines of evidence provide support for the hypothesis, and warrant new research approaches--which we suggest--to address limitations in existing studies. The hypothesis has implications for understanding possible etiologic roles of de novo mutations in autism, and it suggests possible approaches to primary prevention of the disorder, such as addressing widespread vitamin D deficiency and exposure to known mutagens.

There has been remarkable progress over the past 20 years in pushing forward our understanding of many facets of autoimmune disease. Indeed, knowledge of the genetic basis of autoimmunity and the molecular and cellular pathways involved in its pathogenesis has reached an unprecedented level. Yet this knowledge has not served to prevent autoimmune disease nor to curtail the dramatic rise in its incidence over the same interval. Population-level genetic changes cannot explain this trend; thus, environmental factors are strongly implicated. Among the possible environmental contributors to autoimmune disease, air pollution exposure has received very little attention. Although there is only a small amount of published data directly examining a possible causal relationship between air pollution exposure and autoimmunity, data from related fields suggests that it could facilitate autoimmunity as well. If correct, this hypothesis could prove to have sizeable public health implications.

BACKGROUND: Although the association between exposure to particulate matter and health is well established, there remains uncertainty as to whether certain chemical components are more harmful than others. We explored whether the association between cause-specific hospital admissions and PM2.5 was modified by PM2.5 chemical composition.
 
METHODS: We estimated the association between daily PM2.5 and emergency hospital admissions for cardiac causes (CVD), myocardial infarction (MI), congestive heart failure (CHF), respiratory disease, and diabetes in 26 US communities, for the years 2000-2003. Using meta-regression, we examined how this association was modified by season- and community-specific PM2.5 composition, controlling for seasonal temperature as a surrogate for ventilation.
 
RESULTS: For a 10 ug/m3 increase in 2-day averaged PM2.5 concentration we found an increase of 1.89 % (95% CI: 1.34- 2.45) in CVD, 2.25 % (95% CI: 1.10- 3.42) in MI, 1.85 % (95% CI: 1.19- 2.51) in CHF, 2.74 % (95% CI: 1.30- 4.2) in diabetes, and 2.07 % (95% CI: 1.20- 2.95) in respiratory admissions. The association between PM2.5 and CVD admissions was significantly modified when the mass was high in Br, Cr, Ni, and Na+, while mass high in As, Cr, Mn, OC, Ni, and Na+ modified MI, and mass high in As, OC, and SO42- modified diabetes admissions. For these species, an interquartile range increase in their relative proportion was associated with a 1-2% additional increase in daily admissions per 10 ug/m3 increase in mass.
 
CONCLUSIONS: We found that PM2.5 mass higher in Ni, As, and Cr, as well as Br and OC significantly increased its effect on hospital admissions. This result suggests that particles from industrial combustion sources and traffic may, on average, have greater toxicity.

Rationale: Little is known about the long-term effects of air pollution on pneumonia hospitalization in the elderly.

Objectives: To assess the effect of long-term exposure to ambient nitrogen dioxide, sulfur dioxide, and fine particulate matter with diameter equal to or smaller than 2.5 mum (PM(2.5)) on hospitalization for community-acquired pneumonia in older adults. Methods: We used a population-based case-control study in Hamilton, Ontario, Canada. We enrolled 345 hospitalized patients aged 65 years or more for community-acquired pneumonia and 494 control participants, aged 65 years and more, randomly selected from the same community as cases from July 2003 to April 2005. Results: Long-term exposure to higher levels of nitrogen dioxide and PM(2.5) was significantly associated with hospitalization for community-acquired pneumonia (odds ratio [OR], 2.30; 95% confidence interval [CI], 1.25 to 4.21; P = 0.007 and OR, 2.26; 95% CI, 1.20 to 4.24; P = 0.012, respectively, over the 5th-95th percentile range increase of exposure). Sulfur dioxide did not appear to have any association (OR, 0.97; 95% CI, 0.59 to 1.61; P = 0.918). Results were somewhat sensitive to the choice of methods used to estimate air pollutant levels at residential addresses, although all risks from nitrogen dioxide and PM(2.5) exposure were positive and generally significant.

Conclusions: In older adults, exposure to ambient nitrogen dioxide and PM(2.5) was associated with hospitalization for community-acquired pneumonia.

Abstract

Background: A high rate of post-infectious fatigue and abdominal symptoms two years after a waterborne outbreak of giardiasis in Bergen, Norway in 2004 has previously been reported. The aim of this report was to identify risk factors associated with such manifestations.

Methods: All laboratory confirmed cases of giardiasis (n = 1262) during the outbreak in  Bergen in 2004 received a postal questionnaire two years after. Degree of post-infectious abdominal symptoms and fatigue, as well as previous abdominal problems, was recorded. In  the statistical analyses number of treatment courses, treatment refractory infection, delayed
education and sick leave were used as indices of protracted and severe Giardia infection. Age,  gender, previous abdominal problems and symptoms during infection were also analysed as possible risk factors. Simple and multiple ordinal logistic regression models were used for the analyses.

Results: The response rate was 81% (1017/1262), 64% were women and median age was 31 years (range 3-93), compared to 61% women and 30 years (range 2-93) among all 1262 cases. Factors in multiple regression analysis significantly associated with abdominal symptoms two years after infection were: More than one treatment course, treatment refractory infection, delayed education, bloating and female gender. Abdominal problems prior to Giardia infection were not associated with post-infectious abdominal symptoms. More than one treatment course, delayed education, sick leave more than 2 weeks, and malaise at the time of infection, were significantly associated with fatigue in the multiple regression analysis, as were increasing age and previous abdominal problems.

Conclusion: Protracted and severe giardiasis seemed to be a risk factor for post-infectious  fatigue and abdominal symptoms two years after clearing the Giardia infection.

West Nile virus (WNV) RNA was demonstrated in 5 (20%) of 25 urine samples collected from convalescent patients 573–2452 days (1.6–6.7 years) after WNV infection. Four of the 5 amplicons sequenced showed 199% homology to the WNV NY99 strain. These findings show that individuals with chronic symptoms after WNV infection may have persistent
renal infection over several years.

This is the case history of an allergic teacher who developed variable loss of power and co-ordination in her legs following small operations requiring anaesthetics, which later developed into severe multiple chemical sensitivity while she was working in a new building, so that she needed a wheelchair. When full investigation in a teaching hospital failed to find an explanation, she was referred for an allergy opinion, admitted to the Airedale Allergy Centre (AAC), an environmentally-controlled unit, and her symptoms cleared during a therapeutic fast. The environmental factors provoking her symptoms were identified and her management consisted of avoidance and low-dose desensitization. In the 11 years since then she has managed to keep well with care and takes 12-mile walks.

Myalgic encephalomyelitis / chronic fatigue syndrome (ME/CFS) is a medical illness characterized by disorders in inflammatory and oxidative and nitrosative (IO&NS) pathways.This paper examines the role of Coenzyme Q10 (CoQ10), a mitochondrial nutrient which acts as an essential cofactor for the production of ATP in mitochondria and which displays significant antioxidant activities.Plasma CoQ10 has been assayed in 58 patients with ME/CFS and in 22 normal controls; the relationships between CoQ10 and the severity of ME/CFS as measured by means of the FibroFatigue (FF) scale were measured. Plasma CoQ10 was significantly (p=0.00001) lower in ME/CFS patients than in normal controls. Up to 44.8% of patients with ME/CFS had values beneath the lowest plasma CoQ10 value detected in the normal controls, i.e. 490 mug/L. In ME/CFS, there were significant and inverse relationships between CoQ10 and the total score on the FF scale, fatigue and autonomic symptoms. Patients with very low CoQ10 (<390 mug/L) suffered significantly more from concentration and memory disturbances. The results show that lowered levels of CoQ10 play a role in the pathophysiology of ME/CFS and that symptoms, such as fatigue, and autonomic and neurocognitive symptoms may be caused by CoQ10 depletion. Our results suggest that patients with ME/CFS would benefit from CoQ10 supplementation in order to normalize the low CoQ10 syndrome and the IO&NS disorders. The findings that lower CoQ10 is an independent predictor of chronic heart failure (CHF) and mortality due to CHF may explain previous reports that the mean age of ME/CFS patients dying from CHF is 25 years younger than the age of those dying from CHF in the general population. Since statins significantly decrease plasma CoQ10, ME/CFS should be regarded as a relative contraindication for treatment with statins without CoQ10 supplementation.

Veterans of the 1990-1991 Gulf War have been reported to have an increased incidence of amyotrophic lateral sclerosis (ALS) compared to personnel who were not deployed. An excess of ALS cases was diagnosed in Gulf War veterans younger than 45 years of age. Increased ALS among Gulf War veterans appears to be an outbreak time-limited to the decade following the Gulf War. Seeking to identify biologically plausible environmental exposures, we have focused on inhalation of cyanobacteria and cyanotoxins carried by dust in the Gulf region, particularly Qatar. Cyanobacterial crusts and mats are widespread in the deserts of Qatar, occupying up to 56% of the available area in some microhabitats. These cyanobacterial crusts, which help bind the desert sands, are dormant throughout most of the year, but during brief spring rains actively photosynthesize. When disturbed by vehicular traffic or other military activities, the dried crusts and mats can produce significant dust. Using HPLC/FD, an amino acid analyzer, UPLC/MS, and triple quadrupole LC/MS/MS we find that the dried crusts and mats contain neurotoxic cyanobacterial toxins, including beta-N-methylamino-L-alanine (BMAA) and 2,4 diaminobutyric acid (DAB). If dust containing cyanobacteria is inhaled, significant exposure to BMAA and other cyanotoxins may occur. We suggest that inhalation of BMAA, DAB, and other aerosolized cyanotoxins may constitute a significant risk factor for the development of ALS and other neurodegenerative diseases.

Comment:  Studies like this are unfortunate as they don't even address the biological variations seen in people with MCS.  They didn't even test subject diagnosed with MCS, so they were not studying MCS.   They were studying healthy people.  They make so many gross assumptions it shocking how a study like this could be published in a journal.

OBJECTIVE: Multiple chemical sensitivity (MCS) is characterized by a variety of symptoms in response to nontoxic concentrations of chemicals. To further test a laboratory model of MCS based on symptom learning, we used a stronger respiratory challenge in this study than in previous studies to induce symptoms (20% CO(2)-enriched air, unconditioned stimulus). Additionally, extinction of learned symptoms was investigated.
 
METHODS: Diluted ammonia (AM) and butyric acid (BA) served as conditioned (odor) stimuli (CSs). In a learning phase, healthy participants received three breathing trials of BA mixed with CO(2) and three trials of AM mixed with regular air, or the reversed combination. In a test phase, the same trials were administered without CO(2). Immediately after odor onset, participants rated their anxiety and expectancy to experience symptoms during that trial. Self-reported symptoms were assessed after each trial; respiratory behavior was measured throughout the experiment.
 
RESULTS: Only participants who learned to correctly anticipate symptoms (learning phase) reported elevated symptoms in response to the CS+ odor (test phase). Anticipation was more likely correct and learned symptoms were more elevated when BA had been previously associated with CO(2). Across test trials, anticipatory anxiety diminished, but learned symptoms did not. Participants failing to learn the prearranged contingencies reported overall more symptoms and anxiety.
 
CONCLUSIONS: Strong respiratory challenges impede extinction of learned symptoms. Conscious expectancy, which may be modulated by odor quality, determines whether learned symptoms develop in response to a specific odor or to the general context. The results further support a symptom learning account of MCS.

Multiple chemical sensitivity (MCS) is a chronic recurrent condition characterized by reduced tolerance to various environmental agents or to a class of chemical substances. An evidence-based diagnostic panel is not available nor is there a universally accepted definition of the syndrome. This study retrospectively evaluated the prevalence of MCS symptoms and associated risk factors in a sample of 660 young adults, by means of a structured questionnaire. Symptoms of MCS were found to be widely distributed in the sample population and several of the main risk factors suspected to be associated with MCS were not consistently associated with these symptoms in the sample. On the other hand a significant correlation (p<0.05) was found between atopy and symptoms. Further studies are therefore needed to evaluate this association and reach a unifying definition of the syndrome.

Preparing for MCS Awareness Month 2010

Now is the time to start thinking about what each of us as individuals and organizations can do in May for MCS Awareness and Education Month.  It's only 4 months away an it often takes time to contact politicians and community leaders to coordinate events. So, it is a good time to think about what we'd each like to do and create a plan for when to launch it into motion.  Last year, MCS America published an Event Planning Kit.  This kit is full of ideas for ways to promote awareness, including:
